Employment opportunities for nurses are projected to grow at a faster rate (15%) than all other occupations from 2016 through 2026. Our country continues to face nursing shortage due to lack of institution offering nursing education, increase age or retiring population, nurse burnout and violence. According to Parker, 2020, there are few solutions that will help alleviate nursing shortage; some of these are: Use an onboarding program – to make new nurse feel welcome, with this program it helps employees to stay on their job. “A good onboarding program can ease new nurses into the job so they’re less overwhelmed in the first few weeks at a new job. A good example is to avoid assigning new nurses to problem patients right away” (Parker & Officer, 2020), Giving incentives to the nurses – it is nice to consider to give incentives like points systems to those who picked up extra shift, or had a bad run, provide career enhancement such as continuing education and career advancements, and lastly, hospitalizations needs to be flexible to work schedules to accommodate nurses personal and professional needs – some nurses stay away on certain mangnte hospital due to no set work schedules, some nurses prefer to work three day straight so that on their off they can take care of their kids or do household choirs for the family. “More flexibility in shift lengths and start times can give nurses the flexibility to choose options best for their personal obligations ensures they’re happier with their work life balance, promoting greater satisfaction and wellness: (Parker & Officer, 2020).

The largest event that has strained nursing and availability of nurses is the growth in the ageing population. Short term and long term care solutions for the ageing and elderly have grown significantly over the years. For example, two to three million Boomers will age into Medicare every year for the next 30 years (ANA,2020). Many long term care operators are subject to filling the gap with agency or contract nursing which adds additional strain to finances and does not provide a quality system for continuity of care and resources. One method providers utilize is very targeted recruiting and support this process with hefty sign on bonuses and employee retention plans. Other means include teaching and training on the job and promote nursing education while providing nursing assistant careers. Utilizing these resources and promoting a secure workplace environment has assisted in fostering the career path and longevity of nursing the long term care environment.

Nurses are regarded as one the most integral part of the healthcare team. Furthermore, they are designated as patient advocates based on the time and nursing care they provide to patient, keeping physicians informed of the slightest deviations and making sure that optimal and effectual intervention is furnished in accordance with the patient need. According to the World Health Statistics Report, there are approximately 29 million nurses and midwives globally, with 3.9 million of those individuals in the United States ( Haddad & Butler, 2020). However, nursing profession continues to face shortage on a regular basis in spite of receiving professional acclamation due to varied significant reasons, out of which the most highlighted ones are nurse burnout, staffing ratios and violence in healthcare settings. As the patient influx heightens, so does the workload but there is no difference in the staffing ratio. This misbalanced staff patient ratios can prove to be detrimental both for patient and nurses leading to medical errors, frustration, job dissatisfaction and exhaustion. Neither the patient nor the nurse wants to be positioned in this situation but that is what the current situation is realistically. Dissatisfaction with one’s profession can also be detrimental to one’s personal health. In nursing, this is a notable dilemma. Nurses are consistently found to be among the most overworked, stressed, and sick workers, with more than 8% of the workforce absent each week due to illness (Minoritynurse, 2013). Additionally, nurses encounter diverse populace which can sometimes be brutal and abusive, hence putting one at risk of verbal or physical exploitation which force nurses to quit the job.

Top management in tandem with nurse leaders is deploying wide-ranging measures to retain staff and employ more new members. Some of them include sign in bonus, relocation coverage and tuition allowance for continued education. Apart from that, many companies are employing seasonal and travel nurses on contract with lucrative salary so as to fill in the gap temporarily. The resolution to generate a constant progression to the nursing shortage will have to be more practical, receptive to nursing professional concerns and must attend to long-term conflicts. There is no simple solution to the nursing shortage, but as the need for nurses grows more acute, patients and the health care community will feel the pain (Galehouse, 2019).

There are many factors that are contributing to the nursing shortage. Like may occupancies nursing is a multigenerational workforce. According to the American Association of colleges of Nursing over have are approaching the retirement age (AACN, 2020). Nursing schools struggle to have enough faculty to educate nursing students. This leads to the slow rate in meeting the demand for nurses. Working in an environment with insufficient staffing causes many other of its own issues of work stress and low job satisfaction leading to nurses to leave the profession. Baby boomers are getting older, they are an aging population that needs more nurses to meet this demand. There are efforts in place to address these shortages. In 2014, University of Wisconsin set an example when they announced they would fund3.2 million through the UW System Economic Development Incentive Grant that provided fellowship and loan forgiveness to faculty that taught in the state after their graduation (AACN, 2020).

5.2 Cara H

The costs of healthcare and treating chronic diseases has been one of the largest factors for healthcare reform. The Affordable Care Act includes programs now led by the Centers for Medicare & Medicaid Services aiming to improve quality and control cost. (Salmond & Echevarria,2017) Early education and preventative disease management are very targeted to all demographics to assist with lessening the disease state management. Nurses will have an expanded role with this paradigm shift and be able to provide better care systems with education and preventative processes which will reduce the costs of healthcare and lessen the episodic acute care requirements. Even rural areas have developed access with wellness centers that provide care and education for patients. Early detection also plays a role with the trend from disease state management to wellness and prevention.

“The U.S. spends more on health care than all the other wealthy countries. Life expectancy in the U.S lags behind many Americans struggle to pay for health care” (The most expensive health care system in the world, 2020). US have the most expensive health care system in the world. Even we invest a lot of money in healthcare system we continue to have low expectancy, high mortality rather compared to other nations. (National Prevention Council, 2011, p. 6). U.S., which spent an average of $9,086 per person annually, life expectancy was 78.8 years (Commonwealth Funds, 2015). Most Americans have chronic illness that cause mortality in the US such as: cancer, COPD, stroke, diabetes, smoking and heart diseases. The key factor to reduce the expense in our healthcare system is to reduce the incidence of chronic diseases. Affordable Care Act focuses to invest in prevention and public health programs that will improve health and help reduce the health care cost and helps people live a longer and healthier life.

What will help reduce chronic disease and be healthy and productive in life? We need to educate people about having a healthy lifestyle choice such as: eating healthy, exercise daily and living in an environment with less pollution. Nurses have the responsibility in promoting wellness and health to our patients by educating them about the risk factors that can cause chronic illness. We can provide resources, referrals, support groups, therapy that can provide care for them. Nurses should advocate the patient’s well being, such as encouraging them to get immunizations, be up to date on their wellness check ups, avoid drugs, eat healthy and have an active lifestyle. Also, we need to approach them holistically and consider the patient’s and families choices regarding what is health; empower them towards the care and let them assume responsibility for self and disease management. Shifting from disease oriented care to promoting health and wellness care across the continuum is essential and a key to positive/optimal outcomes.
